What are the risks and complications of breast augmentation surgery?

As with all surgical procedures there are potential risks and complications of breast implant surgery. Some are minor, some are major and some are very very unlikely to occur. Complications of breast augmentation include infection, bleeding, implant hardening, implant rippling and palpability, breast implant malposition, asymmetry, scarring, breast implant deflation or leakage Overall patient satisfaction is extremely high and the actual occurrence of a complication are quite low.

What are the advantages of silicone gel implants?

Silicone gel breast implants were recently approved by the FDA for general use for cosmetic breast augmentation. Gel implants have been extensively studied for over 10 years before they were released. Gel breast implants have a softer more natural feel than saline breast implants. In patients with very thin skin they provide a better cosmetic result. They are more expensive and may require follow-up with MRI scans to assess implant integrity.

Can breast implants be rejected?

Both silicone gel breast implants and saline breast implants, which have a silicone shell, are well tolerated by the body. Implants are not rejected as in organ transplantation. However, removal may be indicated due to infection or recurrent severe capsular contracture. The incidence of this occurring is less than 1%.

What size will I be after breast augmentation?

It is not always possible to tell a patient what size she will be after breast augmentation but in most cases with physical examination and the patient’s desires a good estimation can be made. Quality and elasticity of the skin, breast dimensions and previous pregnancies are factors to consider in selecting an appropriate size breast implant. A temporary breast sizer is placed at the time of surgery before selecting the final implant size.

Do breast implants have an expiration date?

Patients often ask if they will need to have their breast implants replaced in the future. Although the manufacturers state breast implants should not be considered a lifetime device, they do provide free lifetime replacement of the implants. Unless a patient has problems with her breast implants such as scar formation or deflation, there is no indication that they should be replaced.

Can liposuction be performed with tummy tuck surgery?

In the majority of tummy tuck cases, I perform liposuction on selected areas of the abdomen, waist and anterior hips to help sculpt and shaped the abdomen. Frequently patients will present with localized fat of the upper abdomen. By minimizing the amount of surgical dissection in the upper central abdomen, liposuction can be safely done to thin the remaining skin and fat of the abdomen. Additional fat removal in the waist region can also be done. By combining these techniques in my standard tummy tuck operation; I am able to offer my patients maximal improvement while minimizing wound healing problems.

Breast Feeding and Breast Augmentation

I am frequently asked by patients undergoing breast augmentation if they can breast-feed should they become pregnant. The surgical procedure of breast augmentation does not affect a woman’s ability to breast-feed but is it my recommendation that patients not breast feed with breast implants. A small number of women will develop mastitis with breast-feeding and I believe this increases the risk of development of capsular contracture and breast firmness. The cause is most likely due to a low-grade infection around the implants. Certainly patients have breast fed their children with no adverse effects but I prefer to error on the side of caution. This is an extremely personal decision that patients must balance the rewards against the risks.

What are the chances that my implant will rupture?

Currently there are two types of implants that are used for breast augmentation in the United States. Saline implants are inflatable and have an overall ten-year deflation rate of 3-5%. The manufacturers offer a lifetime warranty on these implants and will cover facility and anesthesia fees up to ten years after surgery.


Rupture rates on silicone gel implants are more difficult to determine. An asymptomatic leakage may go undetected for years but the gel is usually contained in the scar capsule, which develops around the implant. Currently the FDA recommends regular MRI studies to monitor implant integrity. If a leakage is identified, replacement may be indicated. These implants are also covered by an extended manufacturer’s warranty.

What happens to the umbilicus in a tummy tuck?

I am frequently asked if a new umbilicus is made in a tummy tuck or if it is moved. In a mini tummy tuck, the umbilicus is unaffected and not altered by this surgical procedure. In a full or standard tummy tuck, a surgical incision is made around the umbilicus. The upper abdominal skin is released from its underlying attachments to the muscles and then pulled down like a window shade. A small incision is made in this flap and the umbilicus brought out and sutured in its new location. The umbilicus is not detached or recreated. It is simply placed in a new position relative to the skin that has been removed.

mammograms after breast augmentation

Patients often ask me if they should have mammograms after breast augmentation surgery. I advise all patients to continue to have mammograms on a frequent basis according to their age as advised by their family physician. Patients should inform the technician performing the mammogram that they have breast implants so special displacement views are done to fully evaluate the breast tissue. Rupture of breast implants during mammogram is extremely unlikely.
