Breast Implant Placement --- Subglandular or Subpectoral

Breast implants can be placed either subglandular (behind the breast tissue and on top of the chest wall muscle) or subpectoral (underneath the chest wall muscle). Both approaches have advantages and disadvantages but in the majority of patients, I prefer the subpectoral position due to the lower incidence of capsular contracture and easier performance of mammograms. Some patients may have unsightly implant distortion from muscle contraction with subpectoral placement but this is infrequent. Subglandular placement is sometimes indicated in women who participate in body building expositions.
