Rippling and Saline Implants

Ripples and or waves are a problem sometimes associated with saline breast implants. Due to the low viscosity of the saline fluid, shifting of this volume within the implant shell can form ripples associated with certain positions and gravity. These waves may be palpable but not visible but in other cases may be very apparent. Overfilling of saline implants does decrease this problem but too much implant fill will cause more firmness and with increased implant projection with loss of width as the implant assumes a more spherical shape. Implant rippling can be a significant problem in patients with very thin stretched skin. Silicone gel implants often are a better choice in this type of patient as rippling is greatly decreased because of the thicker viscosity of the gel.

How often do saline implants deflate?

Current saline implants marketed by Mentor Corp. and Inamed have a ten year failure or deflation rate of four to seven percent depending on various studies and styles. It seems that textured implants have a higher failure rate than smooth saline implants. Both companies offer lifetime replacement of their implants for deflation. Depending on the circumstances, each company provides reimbursement for some or all of anesthesia and facility fees during the ten-year period in addition to implant replacement. Saline implants should not be considered lifetime devices however unless problems arise, there is no reason to replace asymptomatic breast implants after a certain period of time.

What the risks and complications of tummy tuck surgery?

As with all surgical procedures, tummy tuck or abdominoplasty has general and specific risks. These include infection, bleeding, fluid collection or seroma, wound healing problems, skin loss and scarring. Anesthetic and blood clot complications are possible but very unlikely but can have life threatening consequences? Other complications include persistent numbness, possibility of revisional tummy tuck surgery and a sub optimal aesthetic result.

Can I exercise immediately after I have a tummy tuck?

Early ambulation is encouraged as soon as possible to minimize the risk of postoperative blood clots, however vigorous exercise is not indicated. Patients will have a certain amount of discomfort from muscle tightening after tummy tuck surgery that is usually performed and this will limit activity for the first few weeks. It will typically be four to six weeks before patients can resume a strenuous workout routine.

What are the indications for facial liposuction?

Liposuction of the face is frequently performed during a facelift but may also be done to treat localized deformities of the neck. Most surgeons try to preserve fat in the face as loss of fat volume occurs with age and overaggressive removal may produce a less than desirable result. Liposuction of the neck helps further define the area under the chin and the jaw line. In some patients tightening of the neck muscles or placement of suspension sutures as described by Giampapa may also be performed. With associated chin retrusion a prosthetic chin implant may be placed. By using fine canulas 2-3 mm in diameter, fat removal can be carefully controlled. Fat may also be located beneath the neck muscles and is sometimes removed under direct vision to further define the submental angle.

Will my face be bruised and swollen after facelift surgery?

Bruising and swelling may occur after facelift surgery. Dr. Linderman employs small drains to help decrease these problems after surgery. Patient age, blood pressure and length of surgery are other factors that can cause more bruising and swelling. In most patients bruising disappears within ten days and swelling recedes.

How tight will my face feel after facelift surgery?

Initial and after dressing removal there may be a feeling of tightness in the face and neck following facelift surgery. This will improve as swelling and bruising recede. Following healing there will be a natural appearance and not a windblown or overdone look from your facelift. Patients appear healthy and rested.
