Breast Lift - Brazilian Mastopexy

Many patients presenting for cosmetic breast enhancement desire improvement in the shape and size of their breasts. Frequently due to weight loss, pregnancy or hereditary influences, they may have various degrees of breast ptosis. Patients with breast ptosis who desire only breast lift and reshaping may not require the addition of a breast implant. Until recently breast lift without augmentation has not always provided the best long-term improvement. Surgeons have tried many procedures over the years to reshape the breast. Most of the older procedures depended on various patterns of skin removal to maintain breast shape. However with time the scars and breast skin would invariably stretch and some ptosis would recur due to the effects of aging and gravity.
Over the last 10 years, it became apparent that in order to maintain breast shape the breast tissue itself required remodeling and the skin had little to do with maintaining shape. In 2001 I had the opportunity to attend a lecture on breast lift surgery presented by Dr. Ruth Graf of Brazil. I became intrigued by her method of mastopexy and the wonderful long-term results that could be produced with her technique.
I arranged to learn this surgery from Dr. Graf and Houston surgeon Dr. Tom Biggs. Over the years, I have made several minor modifications but I am convinced that this technique is the best current answer to maintaining a good long-term result with breast lift. Now excellent long-term results can be produced with breast lift alone without implant placement. I call my current procedure a Brazilian Mastopexy.
How is a Brazilian Mastopexy performed?
First the new location of the nipple is determined and an estimate is made of the amount of lift required and the amount of excess skin to be removed. Skin removal is secondary to internal reshaping and suturing of the breast tissue to maintain a long-term result.
Inferiorly a triangular portion of breast tissue is isolated by dissection to preserve its blood supply. Just superior to this a belt of chest wall muscle is raised. The breast tissue is then passed beneath the muscle belt and sutured high on the chest wall to provide superior fullness and lift of the upper breast and a sort of auto augmentation. The belt holds this tissue in position for a permanent lift. Finally, the lower part of the breast where the tissue was transposed is closed bringing the medial and lateral breast pillars together to cone the breast and produced a youthful lifted result.
Advantages of Brazilian Mastopexy
Because this procedure is accomplished without the use of an implant, associated risks and complication of this additional procedure are eliminated. The breast is soft with a pleasing shape. Good Candidates for breast lift surgery may have any of the following conditions:
Breast are pendulous, but are a size that is satisfactory.
Breasts lack substance or firmness
Nipples and areolas point downward and are below the breast crease.
