Gynecomastia or male breast enlargement is a condition that affects both younger men during puberty as well as older adult males. The condition may due to over development of breast tissue as is common in teenagers or due an accumulation of fat which may occur in older men. In younger men the soreness and enlargement may resolve with time but in some cases surgical correction is indicated.
Some males continue to develop breast tissue so that the breasts become large or begin to sag. This can have devastating effects on the psyche of young men, and can be physically uncomfortable. Restricting social events and exercise may then become the norm in these lives.

Causes
The causes of gynecomastia may also be associated with several issues, including:
- Obesity
- Hormonal disorders
- Side effects of medicine
- Chronic liver or kidney disease
Nearly a quarter of all gynecomastia cases have no clear cause. Heavy use of marijuana may also be a cause, but the data is contradictory.

Procedure
During your consultation with Dr. Linderman, he will determine if the enlargement is due primarily to glandular tissue, fat or combination of both. Ultrasonic liposuction combined with tumescent technique is particularly effective in correcting Gynecomastia. This may be combined with a small incision around the areola, the dark skin around the nipple, to remove residual tissue or to reposition the nipple. Recently this additional incision has been eliminated by the use of an arthroscopic cartilage shaver to remove glandular breast tissue beneath the nipple area. This power assisted instrument is inserted through the incision used for liposuction and has been quite effective in removing residual tissue without added scarring.
The male breast reduction surgery itself takes between one and two hours, and can be done in either an outpatient or hospital setting. Incisions are made near or within the areola and the perimeter of the breast. Breast tissue, fat, and skin are removed from the central portion and sides of the breast. Sometimes the nipple, areola, and the underlying tissue are moved to a higher location. A drain may be necessary during healing.
While there will be scarring, this will fade over time. Breast and nipple sensitivity may be reduced for a period of months. Occasionally, asymmetry may occur which can be corrected with further surgery.
Pain during recovery is controlled with medication and the surgical dressing placed on the area after surgery will be replaced with a compressive garment about a week after surgery. This should be worn for two weeks. Swelling and discoloration will begin to reside after about a week, and stitches will be removed about two weeks after surgery. Patients may return to work a week after surgery and may resume physical activity after three to four weeks, or per Dr. Linderman's orders.
