
Breast Augmentation - Implant and Scar Location

In performing Breast Augmentation various options are available to the patient in choosing where the surgical access scar is placed and how the implant is located in relation to the chest wall or pectoral muscle.
There are four possible breast augmentation incisions. These are inframammary, periareolar, transaxillary and transumbilical.
The inframammary approach places the incision in the fold underneath the breast and in most cases it is well hidden and gives direct access for positioning of the implant.
The periareolar incision is quite commonly used with the incision located as a half circle at the lower margin of the areola. I find that these two incisions work very well for almost all of my patients. The periareolar incision may require a little more dissection and may be limited in cases of very small areola. There is no increased risk of sensation loss with this incision. The normal length of both of these incisions for a saline implant is three centimeters. A silicone gel implant will require an incision about twice this length.
Transaxillary and transumbilical are alternative incisions that may also be used but it has been my experience that other problems can arise with their use.
The other option associated with Breast Augmentation is the position of the implant relative to the chest wall muscle. In the vast majority of cases the implant is place subpectoral or underneath the chest wall muscle. This has the advantage of more tissue coverage of the implant compared to submammary placement with the implant on top of the muscle.
Perhaps the greatest advantage of subpectoral placement and the reason it is used by most surgeons is the documented decreased incidence of capsular contracture. Capsular contracture is scar capsule formation around the implant causing hardness and implant distortion. In a very small group of woman submammary placement may be chosen.
