
Breast Ptosis

Ptosis of the breast is the naturally occurring condition of descent of the nipple areola complex and breast tissue on the chest wall. " Ptosis" is derived from the Greek word "fall". This condition may be a result of aging, weight fluctuation, pregnancy or hereditary influence. Patients presenting for breast augmentation may also have ptosis with or without decreased breast volume.
In Grade I Ptosis the nipple is located at the level of the IMF and Breast Augmentation alone will not only increase volume, but also lift and improve breast shape.

In Grade II Ptosis the nipple lies below the level of the IMF and some type of breast lift or mastopexy with or without placement of implant is indicated. The type of mastopexy and incision used will vary depending on how much the nipple must be lifted. The more lift, the more extensive the incision. That can vary from a Benelli or concentric incision around the areola to lollipop or anchor incision.

In Grade III Ptosis the nipple lies well below the IMF and may point down. A more extensive lift is required with an anchor incision.

Some patients present with adequate breast volume and desire correction only of breast ptosis to improve shape. This is an ideal patient for a Brazialian Mastopexy which moves the patient's own tissue to a higher position without an implant. In combination procedures of breast lift and augmentation, a balance is sought between increasing volume and removing excess skin to achieve the best shape and healing of the incisions. Other factors such as thin skin and stretch marks must be considered.
