However, the truth is that many of the changes that occur are the result of smoking, sun exposure, and the use of harsh drying agents, such as soap or makeup. The good news is that to a certain extent, some of these changes are reversible. Chemical Peel
The exfoliation, or removal, of our skin occurs every day as a normal cellular process. A chemical peel is essentially an accelerated form of exfoliation brought on by the application of a caustic chemical agent to the skin surface. Depending on the type and concentration of the chemical, it may just remove the superficial layer of dead skin cells or it may affect the living cells of the epidermis and the dermis.
For an even more superficial peel, in which only the dead skin cells are removed, we use glycolic acid, an alpha-hydroxy acid. A glycolic peel creates a healthy, refreshed appearance to the skin with no down time because the living skin is left intact. For a medium depth peel, we use TCA in a higher concentration so that the deeper layer of skin (dermis) will be affected. As you heal, the old, damaged skin will be replaced by a layer of new and healthy skin, and a layer of collagen will also form below the skin surface to help tighten and smooth out your new skin. Healing is typically complete within 2 weeks, although some redness may persist for 3 to 4 weeks. A medium depth peel can effectively improve facial wrinkles and sun damaged skin. TCA is particularly effective for darker skinned individuals because it is less likely to cause a lightening, or bleaching, of the skin. In certain instances of severely sun damaged skin with deep wrinkles, a deep peel may be necessary. For a deep peel, we use the chemical agent phenol to penetrate to the deep layer of the dermis. The phenol peel targets deep and widespread wrinkles and extremely rough skin. It is used only in selected cases because it tends to have a bleaching effect on the skin which is permanent. An alternative to the deep phenol peel is Laser Skin Resurfacing.
Laser Treatment
Laser skin resurfacing, using either the CO2 or Erbium laser, is one of the latest developments in the war against wrinkles. This technique produces a controlled tissue injury by a process known as vaporization, in which the laser energy is absorbed by the water in the skin cells. In laser skin resurfacing, the depth of penetration can be precisely controlled by adjusting the power settings and number of applications of the laser. This approach is most effective for patients with severe sun damage, deep wrinkles, and certain cases of acne scarring which would not be affected by superficial and medium depth chemical peels. Healing is typically complete in 2 to 3 weeks, although some redness of the skin, which can be covered with makeup, may persist for several months.
Dermabrasion
Dermabrasion is a form of mechanical exfoliation in which a hand held spinning wheel with a textured surface is used to physically smooth out irregular surfaces and remove the superficial layers of skin. This technique is most useful for improving the appearance of facial scars from trauma or acne. It is also considered by many leading centers to be the procedure of choice for improving vertical lines and wrinkles around the mouth. Healing and recovery time are similar to a medium depth chemical peel. In all skin restoration procedures, with the exception of the glycolic peel, a controlled injury to the living skin is intended. Therefore, these techniques do carry a very small risk of scarring and changes in pigmentation, which increase in likelihood with increased depths of injury.
