Facial Fat Grafting
A useful alternative to temporary fillers is autologous fat harvested from various body areas, which can then be processes and injected in to multiple areas to provide contour filling. Fat is has the advantage of lasting longer and can provide significant volume to improve contour and projection in the aging face. As we age, we lose facial volume that can be replaced by fat injections. As this is the patients own fat, there are no rejection problems and with good take of the fat grafts the results can be more long lasting than temporary synthetic fillers.
Many areas of the face and neck will benefit from facial fat grafting. These include the cheeks, nasolabial folds, and lower orbital rims and eyelids. Fat grafting by Dr. Linderman is an office procedure done with local anesthesia. Fat may be harvested from the buttocks or abdomen. It is then prepared centrifuging the fat to separate it from other components. The fat is then transfer to very small syringes and injected into various areas to deposits small pearls of fat to obtain the best survival of the grafts.
Some patients will have minor bruising after the procedure and pain is minimal and easily controlled with oral analgesics. Survival of the injected fat is usually when small volumes are used. In a number of patients, the procedure may need to be repeated to obtain optimal results and restore lost volume and contour deformities. Injectible Soft Tissue Fillers is a class of mostly hyaluronic acid base chemical compositions that have been used extensively for volume replacement and correction of lines and wrinkles in the face. There are currently numerous products, which are FDA approved for these treatments including Restylane, Perlane, Juvaderm and Juvaderm Ulltra to name but a few. Pore size of the hyaluronic acid has been modified for certain indications such as more volume and recommended deeper placement.
Areas that are commonly treated and have shown excellent correction include wrinkles around the mouth, nasolabial folds, lip augmentation and lower eyelid grooves. Length of improvement from the initial treatment has lasted around six months but with retreatment and using higher volumes of the various fillers at the initial treatment, the results can persist for up to eighteen months.

When these products, originally came on the market, I was somewhat skeptical and conservative in my usage. However with time and experience, I have found them to be reliable, non invasive and as currently used, provide excellent patient satisfaction. I try to combine fillers with Dysport and Botox when indicated to not only treat the cause of some wrinkles but also the result. In addition, I have found that patients get better and more long lasting results when I use two or three vials of my preferred filler Restylane or Perlane to provide more volume restoration. The results tend to be better, my patients are happier and their results tend to last longer. I have found that under treatment with a single vial tends to produce average results without the higher degree of patient satisfaction produced with a greater volume of filler. This has been shown to be true by several clinical studies.
